Notice of Privacy Practices
Collection, Use, and Disclosure of Protected Health Information
In accordance with HIPAA, this notice describes how protected health information (PHI) may be collected, used, and disclosed and how individuals can access their information. The Texas Advanced Computing Center (TACC), in support of authorized projects, may collect and process PHI for approved research or operational purposes as defined by applicable project requirements and governing agreements, or may host and process PHI on behalf of project owners (e.g., Principal Investigators). PHI is used and disclosed only as permitted by law and authorized project scope. Project owners are responsible for obtaining consent, providing required privacy notices, and determining permissible uses and disclosures of PHI where applicable. Individuals have rights regarding their information, including access, amendment, and accounting of disclosures. TACC implements administrative, technical, and physical safeguards to protect PHI in accordance with applicable security standards, including access controls, encryption, and monitoring, and limits access to authorized personnel only.
